Team Member - Additional Fields for sign-up / account

It would be nice to add the following field:
- address (for payment and to display state/region)
- phone number for SMS team broadcasts
- custom questions (ie do you have reliable transportation, do you do ride share, etc)
- second email field to make sure they do not mistype their email when signing up (happens often)
